Fill out initial paperwork through our online confirmation email.

We take all PPO plans with Acupuncture benefits. We are in network with Blue Shield, however some of their plans run through ASH, whom we are not in network with. We can provide a Superbill for reimbursement. As a courtesy, we will check your benefits, please provide us with the details of your plan here. We will get back to you within 24-48 hours.

Wear comfortable clothing that is loose and easy to access above the knee and elbow.

It’s important to eat a good meal before your appointment.
